Use this order form to request the installation of Wi-Fi service into a McDonald’s in Walmart restaurant where

AT&T Wi-Fi service is installed. The McDonald’s Walmart team has obtained approval to install public Wi-Fi in

Walmart locations.

Installing Wi-Fi service entails the installation of one or more Wireless Access Points (WAP) in such a way as

to provide Wi-Fi coverage to the McDonald’s in Walmart customer dining area. The WAP is generally installed

in the seating area above the drop ceiling. A cable that provides both power and data is run from the WAP to

AT&T Wi-Fi equipment in the McDonald’s in Walmart back office area. There is no interruption to connectivity

during the installation.

Pricing Information – Wi-Fi Service Retrofit

Please refer to the table below for the charges associated with the retrofit installation of Wi-Fi service.

Charges are 20% higher in Alaska and Hawaii.

Ordering Instructions

Please follow the steps below to request the installation of Wi-Fi service in one or more McDonald’s in

Walmart locations.

Step 1: Enter the National Store Numbers for the McDonald’s in Walmart location(s) to install Wi-Fi service.

Step 2: Sign and fax all pages of this form to 512-519-6453 or email them to: aws-orders@att.com

Additional Terms

I agree to pay AT&T Wi-Fi the charges for installing Wi-Fi service into the locations I have indicated above. The

charges do not include applicable taxes, regulatory fees and surcharges. By signing this form, I am agreeing to the

If building or work area access is denied to the installation technician which results in an additional installation trip,

I agree that an additional installation and trip fee of $250.00 will apply.
